Updated IEP Notices and Forms
The following forms have been updated and uploaded on the ISBE website.
You can access these forms in Arabic, Chinese, English, French, Gujarati, Korean, Polish, Russian, Spanish, Tagalog, Urdu, and Vietnamese.
- Summary of Performance form was posted Nov. 29, 2021.
- Parent/Guardian Notification of Non-Implementation of Service (34-57O) was posted Nov. 29, 2021. This is a new form that was developed to provide proper parent/guardian notification per the state regulations at
23 IAC §226.220(b).
- Parent/Guardian Consent for Initial Provision of Special Education and Related Services (34-57F) was posted Jan. 13, 2022.
English and Spanish versions of the following documents are now available. Additional languages will be posted at a future date.
- 34-54 IEP forms were posted Jan. 11, 2022.

- Notice of Procedural Safeguards for Parents/Guardians of Students with Disabilities (34-57J) was posted Jan. 11, 2022.
