Special Education Private Facility Code Conversion

Please be advised that a conversion of the current 3 digit Private Facility Code (PFC) to a unique Entity Identification (Entity ID) is scheduled for July 6, 2017.  This conversion will cause the following systems to be unavailable for a period of time on that day:  I-Star, State of Illinois Consolidated Financial Report (CFR) and the Private Facility Search Engine.   The Entity ID is a unique identifier that expands the capacity for assigning numbers to special education operating agencies and private programs.   A conversion crosswalk of the old to new codes can be accessed at https://www.isbe.net/_layouts/Download.aspx?SourceUrl=/Documents/Facility_Code_Conversion_Crosswalk.xlsx  

After July 6, please ensure that the new Entity ID is utilized when completing information in the Consolidated Financial Report system, completing amendments to the Nonpublic Special Education Program Calendar, Non-Public Facility Placement Contract (Form ISBE 19-83), Application for Approval of Private Residential Placement Room and Board Reimbursement Form (ISBE 34-37), Room and Board Claim System, as well as I-Star pupil approvals.  Please note that providers will still include “T” or “R” with the facility Entity ID to designate tuition and residential information in the CFR Schedule 2, Schedule of Programs.

Reminder - Nonpublic Special Education Private Facility Calendars

·        The Division of Funding and Disbursement Services is responsible for processing all nonpublic special education private facility calendars.

·        All 2017-18 calendars were due on or before April 17, 2017.

·        All 2017-18 calendar amendments are due on or before June 1, 2018.

·        Be sure to complete all appropriate sections of the calendar(s).

·        Include all Entity IDs for which the program calendar represents.

·        Do not send calendars directly to your program consultant.  Since the proposed and calendar amendments are shared electronically, please email the calendars as well as any questions to ILPCRB@isbe.net

·        Submit the Nonpublic Special Education Program calendar(s), as the Excel workbook version, not a PDF of the calendar, as the excel version allows the necessary ability to edit and insert comments.
